9.1. Form Usage

Form Usage:

MYPTEZ gives you access to six, printer friendly, form templates. These include:

Employee Acknowledgment Forms for PTA employees

Request for Advance/Payment Authorization

Cash Verification Form

Donation Receipt

Payment Authorization/Request for Reimbursement

Audit Report

1.Select "Forms" from the Main Menu

2.Click on the desired form

3.A separate window will appear, from there you can print the document.
